CARE MANAGEMENT

The AOWN Care Management program is designed to provide identification, coordination, and facilitation of community services to assist elderly to remain in their own homes. This program receives its certification from the Nebraska Health and Human Services, Division on Aging.

Care Managers are Registered Nurses or Certified Social Workers. They conduct a complete assessment of each individual to identify needs, develop and recommend a plan of care, help arrange for appropriate community resources, and provide regular follow-up to assure satisfaction and to identify changing needs. This program does not provide skilled nursing services.

The Care Management Unit, by State statute, must determine the individual’s ability to cost share in its provision. Each person’s monthly income is applied to a sliding fee scale to assess their share. But no individual will be denied service due to their inability to pay.
